Rescheduling or Canceling
If you need to call and reschedule or cancel your appointment, please let us know as soon as possible, so that we can open your appointment time to others.

Your First Appointment
On your first visit, we ask that you bring any pertinent medical record information, such as recent xrays, MRI's other imaging studies or reports, and a list of your current medications.

A referral may be required, depending on the type of insurance you have. If a referral is required, your primary care physician must send us the referral form prior to your first visit.

When you check-in for your appointment please have:

• Your current insurance card(s)
• Valid I.D. (driver's license or other valid I.D.)
• Referral form from primary care physician (if required by insurance company)
• Co-payment required by your insurance company
• All pertinent medical records (MRI films, CT films, x-rays, etc.)
• List of the medications you are taking
